Payment, Refund & Certificate of Attendance Policy
I ______________________ have read and understand the refund policy for the Starfish Mediation Anger Management Course and the policy governing the attainment of a certificate of completion for the course. My signature below signifies my understanding and acceptance of all policies.
Payment Policy: Full payment of $250 is due 1 week prior to the beginning of class to hold your space in the class. NOTE: class size is limited & space will not be reserved without payment. Partial payment arrangements can be arranged. You would need to pay ½ ($125) prior to the start of class and the other ½ ($125) is due by the 4th session. Certificates of Completion will not be issued until the fee is paid in full.
Letters: A letter can be written to authorities prior to the start or prior to the completion of class if the participant is registered and has paid either ½ of the class fee or the full fee.
Refund Policy: A full refund can be obtained if requested up to one week prior to the start of the class. No refund will be issued

If not requested one week prior the first day of class.

If participant fails to show up for class.

If participant does not complete the entire 12 hour session.
Certificate of Completion Policy: In order to obtain a Certificate of Completion for the Anger Management Course a participant must be present for the entire (12) hours in the course & have paid the course fee in full.
It is the responsibility of the participant to ensure that this Certificate (if court ordered) will be accepted his/her Court prior to starting the class.
Attendance:
One 2 ½ hr class session can be missed but must be made-up at the next available class in order to obtain a certificate. The certificate will be presented when the class is made-up & the course completed. If more than one 2 ½ hour session is missed the participant would have to retake and repay for the class to obtain a certification of completion.
